MAKE A COPY OF THE FIRST PAGE OF THIS FORM, AND THEN FILL IT IN, AS PER THE
RECOMMENDATIONS BELOW:
1) Write the name/company name and address of the product owner. It is very important to enter a complete address, so as to use it for forwarding information (by regular mail), with a view to assuring continued airworthiness of your airplane.
2) Write the number of airframe hours, from the airplane logbook.
3) Write the number of hours of engine operation, from the engine logbook. Check if this number is the same as the number shown on the control panel instrument of your airplane. In case of different numbers, mention them both, accompanied by their source (logbook and engine instrument, respectively).
To send the information necessary for assuring continued airworthiness of your airplane in emergency cases, please also consider using some other communication media (fax, email) in addition to the main communication channel, i.e. the regular mail.
