e) Systems and equipment
NOTE: In any event involving a system or component monitored/protected by a warning and/or protection system, it is necessary to declare whether these systems worked properly.
1. General
The general criteria below apply to all systems:
a.
Loss or significant malfunctioning of any system, subsystem or set of equipment when the standard operating procedures, routines etc. could not be carried out in a satisfactory manner.
b.
Pilot’s incapacity to control the system:
-Uncontrolled actions;
-Incorrect and/or incomplete feedback.
c.
Damage or malfunctioning of systems.
d.
Interference within or between systems.

e.
Damage or malfunctioning or protection devices associated to systems.
f.
Any event identifying the potential necessity to use a protection device for unpredictable behavior cases.
2. Ventilation
a. Complete failure of cockpit ventilation.
3. Communications
a. Total loss of communications in flight.
4. Electrical system
a.
Failure of one of the DC or AC electrical distribution systems.
b.
Total failure or loss of more than an electrical generator system.
5. Cockpit
a.
Loss of pilot’s seat attitude control during flight.
b.
Breakdown of any system or equipment in the cockpit.
6. Fire protection system
a. Unnoticed malfunctioning or failures of fire protection systems, leading to a loss or reduction of the fire protection capacity.
7. Flight control system
a.
Asymmetries of control surfaces.
b.
Restriction of movement, rigidity, decrease or time lag of feedback upon actuation of flight controls; discontinuous movement of control surfaces.
c.
Flutters on flight control surfaces, noticed by the crew.
d.
Disconnection or failure of flight control system.
e.
Significant interference with normal airplane control or degradation of flight quality.
8. Fuel system
a.
Malfunctioning of the fuel reading system, causing a total loss of the airplane fuel quantity reading or erroneous such readings.
b.
Fuel leakage that may lead to major loss, fire hazard or significant contamination.
c.
Malfunctioning or failure of fuel system, with a significant effect on the fuel supply and/or distribution.
d.
Incapacity to fully use the usable fuel.
9. Instrumentation system
a.
Malfunctioning or breakdown of any instrumentation system whenever there is a risk of readings wrongly leading to inadequate crew actions on the basic systems.
