On November 3, 2003, about 1025 central standard time, a Cessna 206C, Costa Rican registration TI-AVH, registered to and operated
by Alfa Romeo Aerotaxi, as an air taxi flight, struck trees and collided with the ground while on a takeoff climb from Aerodrome
de Golfito, Costa Rica. Visual meteorological conditions prevailed at the time and flight plan information was not available.
The airplane was substantially damaged, the commercial-rated pilot and the passenger were seriously injured. The flight was originating
at the time of the accident.

returning from a routine re-supply flight to Fossil Bluff, a remote field station situated on Alexander Island
on the Antarctic Peninsula.

On November 8, 2003, about 1059 Atlantic standard time, a Short Brothers Skyvan SC7, Guyana registration,
8R-GMC, registered to and operated by Trans Guyana Airways as a domestic charter passenger and
cargo flight, crashed shortly after takeoff from Georgetown, Guyana. Visual meteorological conditions
prevailed at the time of the accident. The pilot, copilot and three passengers sustained serious injuries, and
a flight attendant and one passenger received fatal injuries. The flight was originating at the time of the
accident.

On November 10, 2003, at 11:45 mountain standard time, a Beech 1900D, N254GL, owned and operated by Great Lakes Aviation,
LTD., received minor damage when it collided with a parked airplane while parking at Denver International Airport (DEN), Denver,
Colorado. The airline transport pilot, co-pilot, and 11 passengers were not injured. Day, visual meteorological conditions prevailed.
The scheduled domestic passenger flight was being operated under the provisions of Title 14 CFR Part 121. According to
the co-pilot, the crew was following the marshalling instructions as issued by the marshaller. The taxi speed was slow and the copilot
stated he was unable to see the left wing clearly. The left winglet of N254GL impacted the right wing of N211GL, another
Beech 1900D. The captain shut down engine #1 and the passengers were deplaned without further incident.

On November 12, 2003, at 0645 central standard time, a Learjet 24D, N77JL, piloted by an airline transport pilot, was destroyed
during a forced landing and post accident fire, after a loss of power in both engines. The flight departed runway 12R (6,997 feet by
100 feet, asphalt) at St. Louis Downtown Airport (CPS), near Cahokia, Illinois, at 0641. The business flight was operating under
the provisions of 14 CFR Part 135 flight and was on an instrument flight rules (IFR) flight plan with North Platte, Nebraska, as the
destination airport. Visual meteorological conditions prevailed at the time of the accident. The pilot and co-pilot sustained serious
injuries. The passengers reported no injuries.
On November 12, 2003, about 2200 central standard time, a Saab-Scania AB (Saab) 340, N442XJ, operated
by Mesaba Airlines Inc., as flight 2984, received substantial damage during a bird strike on approach to
Minneapolis-St. Paul International Airport (MSP), Minneapolis, Minnesota. Visual meteorological conditions
prevailed at the time of the accident. The 14 CFR Part 121 passenger carrying flight was operating on
an instrument rules flight plan. The 2 pilots, 1 flight attendant, and 22 passengers were uninjured. The flight
departed from Eau Claire, Wisconsin, and was en route to MSP where it landed without incident. The airplane
was descending through 4,000 feet mean sea level at 245 knots indicated airspeed when it encountered
an unknown number of geese that impacted the vertical stabilizer.

On November 14, 2003, about 1250 Pacific standard time, United Airlines Flight 805, a Boeing
747-400, N178UA, experienced a tail strike on rotation at San Francisco, California. United
Airlines, Inc., was operating the airplane as a scheduled international passenger flight under the
provisions of 14 CFR Part 121. The airline transport pilot licensed captain, 3 aircrew members,
