According to Evergreen Airlines, while the aircraft was in cruise flight at flight level 400, the right wing overheat light illuminated.
The crew accomplished the appropriate checklist, and then about five minutes later the engine number three and engine number
four B fire loops failed. Soon thereafter an acrid odor was detected by the flight crew, and smoke was seen coming from behind the
P-6 circuit breaker panel. The crew then became aware of a small fire in the fire detection card file, which they extinguished with a
had-held fire extinguisher. The crew then diverted to Leone, France, where an uneventful normal landing was executed. After landing
at Leone, the fire detection card file was removed and returned to the United States for analysis.

On October 3, 2003, at 1800 central daylight time, a Cessna U-206G, single-engine airplane, Bolivian registration CP-2169, was
destroyed upon collision with a house while maneuvering near the town of El Peru, Department of Beni, in the Republic of Bolivia.
The commercial pilot and the sole passenger were fatally injured. The airplane was owned and operated by a private individual.
Visual meteorological conditions prevailed and a visual flight rules (VFR) flight plan was filed the air taxi flight. The flight originated
at a private ranch and was destined for the town of El Peru. Witnesses observed the aiplane flying low over the town when it
collided with the unmarked 75 foot tower. The aircraft then collided with a house in which two inhabitants were fatally injured and
two received serious injuries.

While on approach to runway 34L, SkyWest 3920, a CRJ-100ER registered N431SW, struck a bird. The
aircraft did received damage to the inboard flap, but no injuries were reported to passengers or crew members.
The aircraft landed without further incident at SLC.

On October 4, 2003, at 1450 eastern daylight time, a Boeing 737, N972AN, operated by American Airlines Inc., as flight 847, was
substantially damaged while parked at Gate 26, when it was struck by an unoccupied ground support
van, operated by American Eagle Inc., at the General Edward Lawrence Logan International Airport
(BOS), Boston, Massachusetts. The 2 flightcrew members, 4 flight attendants, and 70 passengers were
not injured. Visual meteorological conditions prevailed, and an instrument flight rules flight plan had
been filed for the flight that was destined for the Chicago O'Hare International Airport (ORD), Chicago,
Illinois. The scheduled passenger flight was to be conducted under 14 CFR Part 121.

IN AFT LAVATORY. FLIGHT ATTENDANTS DISCHARGED A HAND HELD FIRE EXTINGUISHER INTO LAVATORY.
EMERGENCY LANDING IN MAUI (OGG). EVACUATION INITIATED, ALL 8 SLIDES ACTIVATED AND USED. ONE
FLIGHT ATTENDANT SUFFERED SLIDE ABRASIONS. THESE ARE PRELIMINARY FINDINGS.

On October 7, 2003, at 1157 Pacific daylight time, a Boeing 777-22B, N222UA, collided on the ground with
a Boeing 777-200ER, JA709A, at San Francisco International Airport, San Francisco, California. N222UA
was operated by United Air Lines as flight UA809 under the provisions of 14 CFR Part 121. JA709A was
operated by Al Nippon Airlines as flight AL007 under the provisions of 14 CFR Part 129. Both airplanes
sustained substantial damage when the right wing tips of each airplane impacted. No one on board either
airplane was injured. There were 272 occupants on board flight UA809, and 177 occupants on board flight
AL007. The international flights were departing for Osaka and Narita, Japan, respectively. Visual meteorological
conditions prevailed and IFR flight plans had been filed.
At this time of the collision, flight AL007 was taxing behind gate G102 and flight UA809 was undergoing a
pushback operation. Thereafter, the airplanes were towed to their respective gates where the passengers deplaned.
