[10] Describes typical access requirements along with a detailed listing of affected access doors, panels and fairings. Unusual access requirements, such as removal of sealant, bushings, pumps, engines, etc. will also be listed as specified on the DTR Check Form.
[11] Provides a listing of zone(s) affected by the associated inspection.

7.2 DTR Check Form
A basic DTR Check Form is shown on Page 7.0.7. The blocks are used or completed as follows:
[1] ITEM - SSI/DTR Check Form Number.
[2] MODEL-SERIES - DTR Check Form eligible airplane applicability by Model-Series as noted in Section 3.0. This may be shown as any combination of 737-300, 737-400 or 737-500.
[3] TITLE BLOCK - SSI title and location of area to be inspected.
[4] OPERATOR(S) - To be completed by the operator using the appropriate three-letter code. If a group of operators have combined their maintenance programs all applicable operators’ codes should be shown.
[5] NO. OF ELIGIBLE A/C - To be completed by the operator or operators showing the total number of airplanes for which the DTR Check Form will be used.
[6] PLOT OF N vs. Δ DTR - Shows relationship between DTR and repeat inspection interval N. The plot includes a curve for each of the maintenance inspection methods and directions listed in Block 13.
[7] STRUCTURE AND INSPECTION DETAILS - Used to show a sketch of the SSI listed in Block 3. Inspection directions are indicated by an open arrow with a reference number. The arrows indicate the general inspection direction for a given zone(s). All parts of the SSI detail(s) covered by the DTR Check Form and visible from that direction should be inspected. Each of the major details in the SSI is identified. The lead crack location is defined above the sketch. This indicates the assumed crack origin for the SSI and is generally shown as a shaded black area on the sketch. Associated multiple site cracking in the other details or hole locations is also generally shown shaded black. In some cases the lead crack location is the only difference between two DTR Check Forms. This generally occurs when one or more of the inspection directions is obscured causing a change in the critical crack configuration. The cracking patterns shown are the most critical, but are not necessarily the only patterns that can occur. A general visual inspection of visible adjacent structure should be conducted during inspection of a given SSI.
[8] NOTES - Used to note any additional information or special circumstances for the SSI shown in Block 3 such as:
