6.0 SSI DISCREPANCY REPORTING
All cracks involving an SSI or related structure in close vicinity to the SSI shall be reported promptly (within five working days) to Boeing. There is no requirement to report completed inspections if no discrepancies have been detected.
6.1 Operator's Initial Report
Upon recognition of an SSI discrepancy, the following information should be telegraphed to Boeing: Heading: ATTENTION -SSI DISCREPANCY

Airplane Serial Number on which discrepancy was found

Identification of structural item found damaged
.
Flight cycles and hours on damaged item

Location and description of damage including direction and size
.
When discovered and inspection method used

6.2 Operator's Follow-Up Report
All available pertinent data, relative to the structural discrepancy, shall be reported to Boeing within 10 working days. A summary of the information that should be reported is shown in the DISCREPANT STRUCTURE REPORT FORM on Pages 6.0.3 and 6.0.4. Operators may use this form or any suitable alternative which contains the same information.
6.3 Reporting Address
All data and reports dealing with the Supplemental Structural Inspection Program shall be submitted either through a Boeing Field Service Representative or directly to:

6.4 Boeing Follow-Up Action
All adverse SSI reports will be reviewed promptly to determine if there are findings directly applicable to the SSI being addressed. If discrepancies are found to be related to the SSI, the following actions will be taken:

The factual data available on the finding will be reported with appropriate priority to operators and airworthiness authorities.

If required, a fleet inspection program will be developed to obtain additional data necessary to formulate a service bulletin.
.
A service bulletin addressing recommended structural modifications and inspections will be issued.
.
The Supplemental Structural Inspection Document will be revised to eliminate the SSI or a portion of the SSI and reference will be made to the new service bulletin in the Section 9.0 list of Service Bulletins with Flight Safety Addenda/Inspection Programs.
