This paper describes an effort to develop and apply a framework to describe the human-system
interactions associated with several different technical applications. The fundamental elements of
the framework are the PSA model and “plant” state, human failure events, unsafe actions, error
mechanisms, performance shaping factors, and plant conditions. The framework was applied in
the analysis of several accidents (including the crash of Air Florida flight in January 1982) and
proved to be robust and insightful.
